A core charge is a refundable deposit added to the price of a rebuildable part. You pay it up front and get it back when you return your old unit, the "core", to the seller. It exists because the old part has genuine value to a remanufacturer: an alternator, a starter or an engine can be rebuilt and resold, and the core charge is the incentive that gets it back into the chain rather than into a skip. Core charges apply to rebuildable mechanical parts. They do not apply to body panels, tailgates, beds or interiors, which is why nothing on this site carries one.
Both appear on the invoice. The core charge is a deposit, not a fee.

a rebuildable core. There is nothing for a remanufacturer to recover, so no deposit is worth collecting.

Effectively yes. Sellers set a return window, commonly around 30 days, and it is enforced. Miss it and the deposit is usually gone.
It varies by state. Some tax the deposit at sale and refund the tax with it. Ask the seller how they handle it in your state.
